REPLACE EXISTING SEPTIC TANK WITH 1250 GALLON ROTH PLASTIC, AS APPLIED AND AS PER PUTNAM COUNTY HEALTH DEPARTMENT REPAIR PERMIT #R-156-25 1/6/26- Jenna called and he only replaced the baffle not the septic tank. Will send copy of revised PCHD permit. dcp
